The All Important Evidence Label

The Evidence Label is the single most important document contained in the portfolio. It is essential that it is completed correctly. The evidence label tells the scorer the expectations for the student so the piece of evidence can be scored correctly. It is the ONLY information available to the scorer. Make sure all the sections are completed. Make sure to explain any abbreviations, acronyms, etc. Be sure to explain the connection between the activity and selected extended indicator. The evidence label must pass the "stranger test." Write it as though the scorer is NOT a special educator and does not know the student. The scorer must be able to score the evidence independently without needing to seek clarification from the teacher.

 

The evidence label has been extensively revised for the 2009-10 assessments. This is the only version of the evidence label that will be accepted. Go here to see the changes and more hints on how to fill it out correctly.

 

The data sheet on the label itself is optional. However, some sort of data sheet or other proof of performance (video, captioned photo, etc.) must accompany the evidence label. And the data format used must show at least 5 trials or instances of the student engaging in the targetted skill. Check the Teacher's Manual at www.ksde.org for more specific information on data formats.

  • Frequency (how often)
